Power automate issue question

For example, a power automate flow is set up to trigger when a sharepoint document is added, sending it for approval to a specific user. The flow runs successfully but the assigned approver does not receive any email or teams notification.

What could be causing??

    I would first check the permissions that you have with each action connected. I use some type of admin for all my flows just to clear out that part. Then I make a no-reply email that if you send it an email it will delete (only for sending emails). Next I would check the conditions if you have any set that they are not skipping anything. You may need to use the "get item" this gets items more in detail and can make sure its sending. Check condition options set as well and may you need a condition within a condition. Use Chatgpt and show it the error even the free version can help fix this issue pretty fast. Chatgpt is smarter than co-pilot. I wasted months using co-pilot for logic stuff.

    In your environment, have all of the people who need to complete approvals been given access to the environment and been assigned the role of approvals users? According to the documentation this is supposed to happen automatically, Get started with Power Automate approvals - Power Automate | Microsoft Learn. You should first check the permissions the user has. They should have at least what is shown below. 
     
    Also, have you checked the approval action to make sure notifications are enabled and you didn't accidentally set it to no.
