Attachment from form to planner isn't working

Hi everyone and @ChadVKealey,
 
The flow i have in Power Automation from Form to planner works but the attachment section only works sometimes and sometimes it doesn't. This is the error message i got when it's fail:
Please help or give me some advise what to do. Thanks in advance!!

    1) you did not mention that detail which would made the answer obvious :-) so please make sure to sure all relevant details hehe
     
    2) I will say what I ask everyone (and I dont install peoples stuff on a zip), please share the flow in pictures.
     
    What it sounds like to me and I am essentially 100% sure is, you are looping and adding attachments, but in your loop you are doing it incorrectly and accidentally attempting to attach the same attachment
     
    That is the only reason it would give that error and why I said what I said. Not that I thought you were looping, simply that you were mistakenly trying to attach the same file multiple times.
     
    please share some pictures of the inputs etc in your flow, and then if you look in your Flow Run that fails, I bet if you go through the Loop section (using the < > keys) and you look at what you are trying to attach, its the same thing.
     
    But hey I have a delicious hat if I am wrong :-)

    @Michael E. Gernaey, i tested it out that if i only attach one attachment then it will works. The moment someone attach more than one documents then i doesn't work and have that error. The documents name is different will that still affect it?

    Are you verifying before hand that the attachment, with the exact same link (meaning location/name etc) doesn't already exist in the Task?
     
    Also, can you verify if any retries happened. Doesn't look like it, but that would cause this too
