Entity Forms still disabled even with correct permissions

Hello Community, 

I have migrated my power pages from developer website to production environment. My entity form is disabled, I have checked all the necessary permissions and it was correct. It was the same when it was in the previous environment. I have also checked if the settings "disabled anonymous access" it also has the correct settings which is "none of the sites"

I thought that it was because of the migration thats why it was disabled. So I created another power page from the production environment itself and duplicated everything. Unfortunately, the entity form is still disabled. I have also created a new table with all of the same settings and metadata. This is very frustrating because there are close to 0 content online to be able to solve this. The documentation also is not sufficient to solve the problem. What can be causing this issue if every setting is correct? 

I have the permission to create and append for Adminstrators and Anonymous users.

EDIT: the security role for the form setting is set to "Everyone"

    Your Power Pages entity form is showing as disabled, even though all permissions and settings seem correct. This likely means something is blocking access behind the scenes—most often it's a missed configuration in table permissions or site access settings.
    Follow these steps to fix:
    1. Check governance setting
      Go to Power Platform Admin Center → Resources → Power Pages sites → Governance Controls
      Make sure "Disable anonymous access" is set to "None of the sites"
    2. Add table permission
      Go to Portal Management app → Table Permissions → Create a new record
      Select your table
      Permission Type = Global
      Access Type = Read (or Create + Append if needed)
      Assign Web Role = Anonymous Users
    3. Confirm web role exists
      Still in Portal Management app → Web Roles
      Search for "Anonymous Users"
      Make sure it’s Active = Yes
      If it’s missing, create one with exact name
    4. Assign access rule to the page
      Portal Management app → Web Pages
      Open the page where the form is shown
      Scroll to Access Control Rules
      Create a new one:
      Right = Read
      Web Role = Anonymous Users
    5. Clear portal cache
      Open your portal URL in browser
      Add this at the end:
      ?portalcacheclear=true
    6. Check form settings
      Portal Management → Entity Forms → Open your form
      Make sure it is enabled
      Confirm the table name is correct
      Check the form has no broken fields like unsupported lookups or file/image columns
    Try these steps one by one, test after each. If form is still disabled, tell me what part is not working, and I’ll guide next.

    Thanks for your response . Please see answers below: 

    1. It is set to "none of the sites"
    2. Permission is already set. Create+Append for anonymous and administrators.
    3. Web roles exist.
    4. Access control created.
    5. Added it at the end.
    6. I see no entity forms but I assume its "Basic Forms" , everything is correct

    still the form is disabled. but my other form with just the same settings is enabled.

    the odd thing is when I change the language the form is enabled. What could be the problem?

    managed to solve the problem. It was a javascript code that was blocking the form which I forgot I added in the code.
