Difficulty Populating Table with Data

Is someone able to assist with this, please?

I have been finding it difficult to upload ParkingRequest.csv and ParkingInspection.csv. It keeps coming back with an error. 

I have attached some screenshot with the error message.

    Difficulty Populating Table with Data
    I had the same issue using import directly in the table.
    Use Model-driven-app to import the data and the problem was solved.
