You’re seeing that banner because you’re navigating to a page (view/form/custom page) that isn’t currently included in the model-driven app’s Pages list, or your last changes haven’t been published yet. Fix it in the modern app designer: Quick fix (most cases)
Go to make.powerapps.com → Apps → (your app) → Edit.
In the left Pages pane, check that the page you’re trying to open (e.g., Loans view, Loans form, Custom page, etc.) is listed.
If it’s missing: click + Add page → choose Dataverse table (or Custom page/Dashboard as appropriate) → pick the specific views and forms you need → Add.
Click Save, then Publish (top right).
Retry the app.
If it’s listed but still shows the banner
Select the page in the Pages list and ensure Include in navigation is enabled (so it’s on the sitemap).
If you recently edited tables/forms/views, go to Solutions (if you’re using one) and click Publish all customizations.
Clear browser cache / hard refresh (Ctrl+F5) or open in an InPrivate window to rule out a stale client cache.
If users (not you) see it: confirm the app is Shared with them and their security roles allow access to the tables/views/forms you added.
