Unable to load data into dataverse table and personalized setting not working in model driven app

I have been finding it difficult to load parkingrequest.csv and parkinginspecetion.csv data into dataverse table. It comes back with an error when ever I tried loading the data.

I tried loading the data form model driven app but whenever I click on 'import csv' it was not working. I also noticed that an error comes up whenever I click on personalized setting on model driven app.

Can anyone help with these please?

When importing via Model Driven App, please use Play mode instead of Edit mode
