You should start at MS Learn and go through all the tutorials for Power Automate, Power Apps, Dataverse, etc.
Once you start learning, the MOST important thing is being willing to do the hard work, don't just post and constantly ask people to solve your problem or build it for you (its a habit people get into).
Learning how to Search for things like, How do I build an MS Form, How do I capture MS Form Responses, etc etc :-) Feel free to try to build stuff and come back here so we can help fill in the educational gap :)
I'm wanting to build a Performance Evaluation form that will track data.
For this I think you mean, you want to build a Form, that you will track its data, not that a form will track data as it cannot.
I mean also, for this, it sounds like
1) An MS Form
2) A Power Automate Flow to capture Form Respones
3) Probably SharePoint, to capture the data from Step 2
You can even send an email after you record it, or use scheduled or triggered Flows (aside from this one) to do Score Roll ups for people to managers etc etc
